Severe Bone Fracture,Prolapsed Intervertebral Disc, Spondyloarthritis and Panfacial Fractures

Severe Bone Fracture: A bone fracture is a medical condition where the continuity of the bone is broken. A significant percentage of bone fractures occur because of high force impact or stress.
Prolapsed Intervertebral Disc: A prolapsed intervertebral disc is a technical term for what is commonly called a slipped disc. These discs are placed between each of the vertebrae of the spine and pose as shock absorbers. Not only will a slipped disc cause pain in the area of the disc, but also in regions that the nerve controls, such as an arm or a leg.

Spondyloarthritis: Spondyloarthritis is the term for a group of inflammatory diseases that cause joint inflammation, or arthritis. Most inflammatory diseases are thought to be hereditary. So far, there isn’t any scientific evidence to suggest that the disease can be prevented.
Panfacial Fractures: Panfacial fractures are defined as multiple fractures of the facial bones that usually affect the upper, middle, and lower areas of the face. The management and treatment of panfacial fracture is an intricate task because of the lack of reliable developments. Panfacial fractures are often termed as maxillofacial injuries. The treatment of patients suffering from injuries require immediate operative management.

Symptoms

Severe Bone Fracture Symptoms

The signs and symptoms of a fracture vary according to which bone is affected, the patient’s age and general health, as well as the severity of the injury. However, they often include some of the following:

  • Pain
  • Swelling
  • Bruising
  • Discolored skin around the affected area
  • Angulation – the affected area may be bent at an unusual angle
  • The patient is unable to put weight on the injured area
  • The patient cannot move the affected area
  • The affected bone or joint may have a grating sensation
  • If it is an open fracture, there may be bleeding

When a large bone is affected, such as the pelvis or femur:

  • The sufferer may look pale and clammy
  • There may be dizziness (feeling faint)
  • Feelings of sickness and nausea.

Prolapsed Intervertebral Disc Symptoms

Symptoms occur in the back and the area under the nerve root. For example, a slipped disc can arouse pain in the lower back and legs.

  • Back pain.
  • Nerve root pain
  • Cauda equina syndrome (nerve root is aroused by a prolapsed disc).
  • Pins and needles.
  • Uneasiness in part of a buttock, leg, or foot

Spondyloarthritis symptoms

The main symptoms of spondyloarthritis are pain, stiffness, and swelling. Bone damage may also occur. Where you feel symptoms in the body depends on the type of spondyloarthritis you have.

  • AS pain often starts in the buttocks and lower back. It may spread to the chest and neck. Tendons and ligaments may also be involved. In rare cases, AS will impact the heart and lungs
  • Enteropathic arthritis may cause pain in the spine, arms, and leg joints. It may also cause bloody diarrhea and abdominal pain due to inflammatory bowel disease
  • Juvenile arthritis often causes pain in the pelvis, hips, ankles, and knees. The condition may also cause fatigue.
  • Psoriatic arthritis can impact the spine. When this happens, it’s known as psoriatic spondyloarthritis. It may also cause pain in the neck.
  • Reactive arthritis may cause inflammation in the urinary tract, the joints, and the eyes. It can lead to inflammation of the spinal joints.
  • Undifferentiated arthritis often causes symptoms similar to AS. This includes pain in the lower back, buttocks, and heels.

Panfacial Fractures Symptoms

Panfacial fractures involve injuries to the upper, lower, and middle thirds of the facial bones. Such injuries often result in multisystem injury, requiring team approach. The injury can be identified with early symptoms which include:

  • Difficulty in breathing, in case of nasal bone fractures
  • Swelling around the eyes and the traumatized area
  • Internal bleeding

Treatments

Severe Bone Fracture Treatments

Bone healing is a natural process-tTrusted sSource which, in most cases, will occur automatically. Fracture treatment is usually aimed at making sure there is the best possible function of the injured part after healing.
Treatment also focuses on providing the injured bone with the best circumstances for optimum healing (immobilization).

For the natural healing process to begin, the ends of the broken bone need to be lined up – this is known as reducing the fracture.
The patient is usually asleep under a general anesthetic when fracture reduction is done. Fracture reduction may be done by manipulation, closed reduction (pulling the bone fragments), or surgery.

  • Immobilization – As soon as the bones are aligned they must stay aligned while they heal. This may include:
  • Plaster casts or plastic functional braces – These hold the bone in position until it has healed.
  • Metal plates and screws – Current procedures may use minimally invasive techniques
  • Intramedullary nails – Internal metal rods are placed down the center of long bones. Flexible wires may be used in children.
  • External fixators – These may be made of metal or carbon fiber; they have steel pins that go into the bone directly through the skin. They are a type of scaffolding outside the body.

Prolapsed Intervertebral Disc Treatments

Conservative treatmen :-
Physical exercise is important to repair the prolapsed intervertebral disc. If the pain is debilitating, normal activities may not be possible.

Medications :-
A combination of anti-inflammatory and paracetamol-based medications are usually the best, together with physiotherapy.

Surgery :-
Physical treatment might include surgery and nerve sheath injection if the former has shown better recovery. The treatment offered to each patient is tailored to their clinical presentation and radiological findings.

Spondyloarthritis Treatments

There’s no cure for spondyloarthritis. Treatment focuses on reducing pain, improving or maintaining mobility, and reducing your risk for complications.
Although it may seem counterintuitive, regular movement is critical in managing discomfort associated with the condition.
Treatment plans are individualized, but most will include:

  • Physical therapy
  • Low-impact exercise
  • Non-steroidal anti-inflammatory drugs
  • Corticosteroid injections
  • Antirheumatic drugs
  • TNF alpha-blocker drugs

Panfacial Fractures Treatments

Grafting :-
It is used to reconstruct large bone defects. On the contrary, discontinuity defects are treated using maxillomandibular fixation (MMF).

Top-to-bottom technique :-
Involves treating the face in three stages following the sequence of the outer facial frame, then the inner facial frame, followed by maxilla reconstruction. If required MMF is performed at the end

Bottom-to-top technique :-
Uses screws and plates for repairing mandible fractures, and the rest of the face is treated as an isolated midface fracture.
